A recovery oriented day program and outreach service

Outreach Services

What the Amaroo Outreach Service does
The Outreach Service provides home based outreach to those who seek support in their own living environment and supported accommodation for those who require housing and support. Clients receive weekly home visits. All clients of the outreach service develop an individual recovery plan with their outreach worker and this plan forms the basis of the work that the outreach worker does with the client.

What the Outreach Service aims to do

The Outreach Service aims to support clients in their recovery and is based around individuals’ support needs. As such each client receives a service tailored around their individual recovery goals. Typically this might include supporting clients to; maintain a safe environment, learn new skills, gain greater access to their community, form new relationships and maximise their independence.

Who can get support from the Amaroo Outreach Service

The Outreach Service is for adults 16 – 64 yrs old who live within the City of Hobsons Bay and experience mental health issues.

Where

The Home Based Outreach Service provides services to people living anywhere within the City of Hobsons Bay. Supported housing outreach properties are located throughout the City of Hobsons Bay in Williamstown, Newport, Altona, Spotswood and Altona Meadows.

How to access the Outreach Service

Access to the Outreach Service is by referral only including self referral. Clients referred to the outreach service all undergo a formal assessment process. For supported housing this process will include a visit the property and an opportunity to meet the other tenant if it is a share property.
