A Youth Residential Rehabilitation Service for young adults 16 to 24 years who have experienced serious mental health issues
Program Description
The Jacaranda Program is a Youth Residential Rehabilitation Service for young adults 16 to 24 years who have experienced serious mental health issues within the West Hume Region. Residents may stay for up to 2 years at the program. Some fees are charged for the residential program. Staff are in attendance at the program from 7am to 9pm with on call phone support available between 9pm and 7am.
Jacaranda’s physical location is close to the CBD, shops, public transport, the Victoria Lake, walking tracks and an indoor aquatic leisure centre. The program consists of 3 fashionably furnished, stand alone units, fully equipped with electrical equipment and all other necessities including crockery and linen. Each resident has their own bedroom and shares the living areas with two or three others.
Jacaranda has an Outreach Service available for approximately 4 young people aged 16-24 years of age. This service is offered to new referrals of young people who are considering entry into the residential program and also include assisting clients moving on from the residential program.
Program Approach
Choosing to become part of the Jacaranda community means making a commitment to be involved with others, to actively participate in learning more about yourself, your strengths and to discover where the choices you make today will lead you in the future.
Each resident works with a Key Worker on an individual plan that expresses what they want for their future and what steps and skill development is necessary for them to take to get there. There is an emphasis on assisting residents deal with day to day tasks and preparation for independent living. There are also fun social and recreational opportunities and many positive links have been made with the local community, education and employment support services
Selection Criteria
Outreach and Residential Services are available to young adults aged between 16 and 24 with mental health issues who reside within the West Hume Region, and are willing to commit to making positive change in their lives.
Referral Procedure
Referrals are accepted from all agencies in the region and self referrals from young people. Prospective participants undertake an informal visit to the program first to meet staff and residents. Referral forms are then completed and submitted before an interview and assessment date is confirmed and takes place.
