Mental Health Accommodation Pathway at Discharge (MHAPD)
What the service does:
The MHAPD program provides a service to people who are identified as homeless, at risk of, or experience recurring homelessness. MHAPD staff engage and support clients to address their accommodation needs prior to discharge from a mental health facility within the Metropolitan Central East Region.
Who can receive a service:
People aged between 15 and 64, in a clinical mental health facility, identified as needing assistance to obtain and maintain accommodation.
Where is the office:
The MHAPD office is Nunawading. MHAPD staff outreach to Upton House, CCU and SECU clients.
What the program aims to do:
The MHAPD support staff work alongside clients to identify support needs, explore accommodation options and access the most appropriate accommodation available. MHAPD is a short term service providing intensive case management in order to help prevent clients from re presenting to inpatient services.
How to access the service:
A referral to MHAPD is made from one of the three mental health facilities once it is identified that a person requires assistance to access accommodation and support prior to discharge.
