Trinity Community Support Services Services for adults 16-64 years recovering from mental illness.
What Trinity Community Support Services does: Trinity Community Support Services offers a range of outreach and centre based recovery focused rehabilitation and support programs to 160 people who have a disability as a result of mental illness. We have nomination rights for 13 long-term housing properties as well as providing service to people who are in public or private housing within their own communities as well as those people who are homeless or at risk of homelessness.
Where: Our office is located in Wangaratta and our services are provided across the Rural Cities of Wangaratta and Benalla, the Shires of Alpine, Mansfield and Moira east, in the Central Hume region of Victoria
The Psychosocial Rehabilitation Day Program Where: The PSR Day program is based in Wangaratta and provides a range of educational, recreational, prevocational and social activities designed to enhance the skills, well-being and community integration of participants while assisting with the attainment of life goals and personal challenge in a safe and secure environment. All programs are developed with participant input and are reviewed biannually.
What: Currently the Day program offers a number of diverse activities Monday- Friday such as Art, Jewellery making, Cooking and menu planning, Mens and Womens groups, Handipersons service (pre-vocational) and counseling/self development, educational and social/recreational groups. Activities are offered on a term by term basis and a monthly calendar of events is published and circulated to interested parties and services.
How to access the program:Referrals can be made by GP’s, Case Managers, Family and prospective Clients. Referrals are accepted on the SCT tool and intake and eligibility is assessed at our fortnightly team meeting.

Day to Day Living Program
What: Aims to support people aged 16-64 years with a severe and persistent Mental Illness, who experience social isolation, to develop stronger community links that increase their ability to participate more independently and inclusively in social, recreational and educational activities within their own community
Where: The service is based at Trinity Community Support Services in Wangaratta and provides group programs across the Central Hume region of Victoria. This program offers term based structured and drop-in programs to people in the townships of Wangaratta and Benalla and structured programs in Yarrawonga and Myrtleford. All programs are delivered in the clients own communities utilizing community based facilities and supports.
