Sarah's Story

Trinity Community Support Services

Services for adults 16-64 years recovering from mental illness.

What Trinity Community Support Services does:

Trinity Community Support Services offers a range of outreach and centre based recovery focused rehabilitation and support programs to 160 people who have a disability as a result of mental illness.
We have nomination rights for 13 long-term housing properties as well as providing service to people who are in public or private housing within their own communities as well as those people who are homeless or at risk of homelessness.

Where: Our office is located in Wangaratta and our services are provided across the Rural Cities of Wangaratta and Benalla, the Shires of Alpine, Mansfield and Moira east, in the Central Hume region of Victoria

Pathways Program.

What: 
This program is a partnership between Eastern Hume Housing and Mental Health services which aims to provide pathways for up to 13 people with Mental Illness who are homeless or at risk of homelessness and to assist in the development or enhancement of community living skills and community integration to support ongoing safe and secure housing options.

Where:
The service is based at Trinity Community Support Services in Wangaratta offering housing support to people in Eastern Hume who  have been identified as homeless or at risk of homelessness and are leaving the Acute Psychiatric In patient Unit at Northeast Health in Wangaratta.

How to access the program:

Referrals can be made by GP’s, Case Managers, Family and prospective Clients.
Referrals are accepted on the SCT tool and intake and eligibility is assessed at our fortnightly team meeting.
