What Wodonga Home Based Outreach does
Wodonga Home Based Outreach offers a recovery focused service for 46 people, 16 – 64 years who have a disability as a result of mental illness and the skills to live in the community with outreach support. We have nomination rights to 15 long-term properties and also provide service to people in their own housing and to those who are homeless.

The Wodonga Home Based Outreach office is situated in central Wodonga and services Wodonga and Indigo shires.
What the program aims to do
The program aims to promote recovery through recognition of existing strengths and the development of an individual plan that may include areas such as
- Reconnection with the community and family
- Accessing education, employment and recreational activities
- Development of supportive networks
- Self management and independence
- Social skill development
- Daily living skills
- Community access
- Physical health
- Spiritual needs
How to access the program
Discussion with the staff to ascertain eligibility, waiting list and referral forms. Referrals to the program can be made by self, family, health and other professionals. Referrals are assessed fortnightly by the team and the referring person notified of the outcome.
