Connect2
An intensive home-based outreach service for homeless people with a psychiatric disability.
What Connect2 does:
The Connect2 program provides intensive support to people who are homeless and have significant mental health issues. Connect2 clients could also have a history of homelessness or transience, and may be unable to gain access to the necessary services in order to address their issues.
Connect2 Clients:
· Are single people aged 16 to 64.
· Have links to the metropolitan central east region of Melbourne.
· Have an established pattern of homelessness.
· Experience a mental illness and have complex needs.
· Move frequently between boarding and rooming houses.
· Live in accommodation that is inappropriate for their age or needs.
· May be shelterless or have no fixed address.
Where
Connect2 services the Central East area that incorporates Whitehorse, Manningham and most of Monash. The Connect2 office is located in Nunawading.
What the program aims to do:
Many of the people who are supported by Connect2 experience isolation, either from family and friends or need assistance to access services in the community.
Connect2 aims to provide the necessary support to people in order for them to address their homelessness issues and gain access to appropriate services such as mental health, physical health, legal or family services. The program also aims to address socialization difficulties and connect with the wider community.
How to access the Program:
Referrals to Connect2 are by health professionals or other service providers, however self-referrals and those from family, friends and carers are also accepted.
