People in Victoria’s Great South Coast region can now get more streamlined support for their mental health and wellbeing through the new Mental Health Multidisciplinary Services Team (MDT) program.

This new program positions mental health clinicians at primary care services within the local community, to provide integrated and coordinated care for people living with complex mental health needs. 

This approach allows different support providers to work more closely together, to create a more connected and seamless experience for people accessing support.

The mental health team will be comprised of a care coordinator and allied health clinicians, helping to ensure that people with complex mental health needs receive the care coordination, psychological therapies and psychosocial support they need and are safely supported within their local community. 

People can receive this support for up to two years, depending on their needs and preferences.

Mind Australia has been chosen to deliver the Mental Health Multidisciplinary Services Team program in the Corangamite Shire, Glenelg Shire, Moyne Shire, Southern Grampians Shire and City of Warrnambool LGAs. The program is funded by the Australian Government via the Western Victoria Primary Health Network (WVPHN).

Mind’s Executive Director for Victorian Operations, Belinda McCullough, said the Mental Health Multidisciplinary Services model reduces the need for people to repeatedly share their stories and symptoms when seeking support.

The Mental Health Multidisciplinary Services Team program is designed for people who:

  • experience severe and/or complex mental health challenges
  • experience episodic or persistent mental illness
  • are significantly impacted in their day-to-day lives, or
  • may be experiencing social or environmental stressors related to their mental health and wellbeing.
