NDIS Support Coordination
Mind Australia’s NDIS Support Coordination services can help you get the support you need to live life your way.
The introduction of the National Disability Insurance Scheme (NDIS) was the first opportunity for many Australians with mental health and wellbeing concerns to purchase support that offered them help, hope and purpose – a chance to live independently and participate in the community.

Wellbeing4Life – Australia
Mind Australia’s Wellbeing4Life service provides accessible multidisciplinary care for eligible Private Health Insurance members with mental ill-health. Mind provides face-to-face or telehealth, based on the client’s location, personal preference and practitioner availability.
About Wellbeing4Life
Mind Australia’s Wellbeing4Life service is based on a holistic, psychosocial support model that includes telehealth and face-to-face outreach for eligible Private Health Insurance members. The service is intended for those currently in the private mental health system who would benefit from short to medium term community-based recovery support.
Wellbeing4Life provides an alternative to a psychiatrist-led, hospital-based model of care. The service provides support to individuals who have had a recent hospitalisation related to mental ill-health. The service works to prevent future relapse and hospitalisation by supporting clients to work towards goals that are meaningful and achievable.
Mind’s Allied Health practitioners who deliver Wellbeing4Life include occupational therapists and psychologists. These practitioners deliver assessments and therapeutic support using Mind’s My Better Life tool to support clients in achieving their recovery goals.
Mind’s Allied Health team works as part of a collaborative care team with practitioners from a client’s existing care team, including clinical and non-clinical services. The team also supports the client to engage other community based or clinical supports as required to ensure optimal outcomes.
The service is successful in reducing hospitalisations, the severity and duration of mental ill-health symptoms and promotes mental wellbeing. A total of 87.5 per cent of Wellbeing4U clients said they were satisfied with the service, and felt safe and listened to.

What do our Wellbeing4U clients say?
“I am more resilient and it is easier for me thanks to the strategies I now have. I liked how the sessions went from frequent support to less frequent support (weekly, fortnightly, monthly) as this helped me to try things out on my own and still have support.”
