Supported Independent Living - MELTON SOUTH

At this residence, 24/7 support is provided by Mind Australia as the nominated support provider.  The staff team have specialised skills in supporting people with mental health-related disability and complex needs. They'll help you with daily life tasks and develop your skills for independent living - including helping you to connect with activities and other supports in the community. Support is tailored to help you achieve the goals in your NDIS Plan. Tenancy is managed separately by a third party agent.

Site description

This four bedroom home in a new estate in Melton South is looking for NDIS participants with ‘Robust’ or ‘Improved liveability’ SDA funding. The home is built to a 'Robust' design and at Platinum quality level.  The property has separate facilities for the support staff.

Residents will also require adequate NDIS funding (i.e. Supported Independent Living) to cover the cost of one-to-one and shared supports provided within the home.

Who for:

People whose capacity to live independently is impacted by long-term mental ill-health and who have an NDIS Supported Independent Living package, funded for assistance with activities for daily living and Specialist Disability Accommodation (SDA) at a Robust or Improved Liveability level.

How to access:
