Virtual Step Up Step Down service - ROCKINGHAM
Online video and phone support
Online video or telephone support is now available 7 days a week between the hours of 9:30 am - 5:00 pm, delivering a virtual Step Up Step Down model of support for up to 30 days for each time referred.
Designed for those who have to self-isolate, quarantine, live a long way from mental health services, are coming home from hospital or at risk of social isolation, placed on a wait-list to enter or are being discharged from the Step Up Step Down service.
To participate you need to have access to a computer, tablet or mobile phone and be able to access an internet connection. You should have a private space to have the video/telephone consultations.
What does this service deliver?
A person centered recovery approach including assessment of your needs to manage mental and physical health; one-to-one support sessions to identify and work on your recovery goals; development of a safety and wellness plan; assistance to refer to appropriate services to address alcohol and other drug issues, family and social relationship issues, and housing issues; assistance to gain access to education or employment opportunities, and identify and engage in meaningful activities.
Using our My Better Life plan® tool, you will be supported to identify your support needs and establish goals for your recovery. Our practitioners are highly skilled and will give you practical and motivational support to help you improve your mental health.
They can also connect you to many other health and community support services in the local area.
Mind will work with your family and other important people in your life if you wish and it helps you in your recovery.
Personal one-to-one support and group activities
One-to-one sessions are the main form of support and we also encourage participation in online group activities offered by Mind (depending on availability of spaces) where you will meet others and learn useful skills for better health and wellbeing.
