Transitional Recovery Program (TRP) – Rockhampton

The Transitional Recovery Program (TRP) is a residential support service for people who have been accessing long or short term acute care at Rockhampton Hospital and are ready for discharge, but require extra support before returning to independent living. 

About the service

The Transitional Recovery Program uses practical and therapeutic approaches to support people to build their independence, stabilise and self-manage their mental and physical health, set goals, and engage successfully in their local community.

Participants will receive support for up to three months in a residence in Rockhampton, where they will be able to access up to 24/7 support as needed.

If further support is required, they can receive up to nine months support at a second residence.

Up to six weeks of outreach support will be provided to participants once they exit the program, to further support their transition to independent living.

This service for is for people aged 18+, who are currently accessing long or short-term acute care at Rockhampton Hospital.

How to access

Central QLD Hospital and Health Service will facilitate referrals into this service. Participants must be involved in acute care at Rockhampton Hospital.
